NEW flightform for Flights

I have taken the flightform and made it into a form you can fill out online or on your computer. I made it originally for myself, but sent a copy to Debi if she wished to post it. All the fields are still exactly where they were before. I wanted to make it easier to fill out things that can be filled out before hand and not by hand each time. if anyone wishes, I can make you a personal copy that defaults all the personally related fields like you name, phone number, aircraft ID, rental rate, etc. and email it back to you.

What format (.doc, .pdf, ...) is the file?

I don't why the upload doesn't work. Something else for the todo list.
