Vaccinations: UTD
Health Certificate: YES
Spay/Neutered: YES
Special needs: none, other than surgery was done a week ago

Name of Receiving Rescue/Parties: Operation Roger Truck Driver whose taking her to Oklahoma!! <3

Additional Info/situation: this is urgent! if we can get her there by wednesday shes on her way home from a kill shelter all the way to oklahoma! Please call Annie PLEASE in addition to replying because of the urgent nature of this flight!! thank you guys!! <3
